Registration That Gets It Right the First Time
The Challenge
Technical denials were rising due to incomplete data and missing documentation during preregistration and across multiple registration areas. When gaps weren’t caught early, staff were forced into time-consuming rework later—often after discharge—slowing billing, delaying cash, and creating inconsistent encounter quality.
Key breakdowns included:
- Forms selection was manual and error-prone
Registrars had to decide which forms applied to each visit, leading to missed or incorrect documentation. - Unsigned forms slipped through
Patients frequently left without signatures, and escalation workflows were not consistently in place to resolve issues before discharge. - ID and insurance capture was inconsistent
Photo IDs and insurance cards were photocopied and stored in physical charts, producing variable image quality across registration locations. - Patients were asked for the same information repeatedly
Even when seen recently, patients were re-asked for documents and information already on file—creating frustration and lowering satisfaction.

The client needed a solution that would standardize intake, enforce completeness, and help staff capture the right documentation the first time—every time.
The Solution
We implemented a standardized intake workflow designed to ensure complete, accurate documentation at the point of registration—while reducing redundant requests and downstream rework.
Core capabilities included:
- Document Capture (ID, insurance, supporting paperwork)
Photo IDs, insurance cards, and patient-provided documents were automatically captured via desktop scanner or shared multifunction devices and stored digitally with consistent quality and indexing. - Forms Automation with eSignature
Required electronic forms were automatically selected, prepopulated, and presented to the patient for electronic signature, reducing registrar guesswork and improving compliance. - Auto-Recall of Prior Documents
Using business rules and a deficiency engine, the system automatically recalled prior document links and surfaced what was already on file—eliminating repeat requests for items like photo ID or privacy notices. - Version Control Enforcement
When a form changed, the system detected the new version requirement and automatically enforced capture of an updated, signed form at the patient’s next visit—preventing outdated documentation from persisting.
The Impact
The result was a more predictable, standardized registration process that improved both revenue integrity and patient experience.
- Streamlined front-end operations through automation and reduced manual decision-making
- Reduced technical denials by closing documentation gaps before they reached downstream workflows
- Enforced consistent standards across all registration areas, producing reliable, repeatable encounter quality
- Less rework after the fact, freeing staff time and improving throughput
- Improved patient satisfaction by eliminating unnecessary repeat requests and minimizing friction at check-in
