The Challenge
A B2B partner engaged us to help automate inpatient admit notifications for one of their healthcare clients.
The client was facing increasing payer scrutiny around inpatient admissions. Several payers required formal notification via a 278 admit notification transaction, and failure to comply could result in full denial of payment. Other payers enforced similar requirements but accepted notification via fax or phone call instead.
The client’s process was almost entirely manual.
Key issues included:
- Manual tracking of inpatient admissions using spreadsheets and post-it notes
- Inconsistent notification timing, creating compliance risk
- Multiple payer notification methods, requiring staff to remember different rules
- No automated confirmation tracking, leaving uncertainty around whether notification had been successfully submitted
- Revenue exposure if a required notification was missed
The organization needed a way to ensure that every inpatient admission triggered the appropriate notification—automatically and consistently—without increasing staffing burden.
The Solution
We developed an automated admit notification workflow that monitored real-time admission activity and initiated the correct payer notification based on configured rules.
HL7-Driven Automation – the system listened for HL7 ADT (Admit, Discharge, Transfer) messages and identified qualifying inpatient admissions in real time.
278 Admit Notification Transactions – for payers capable of accepting electronic transactions, the system automatically generated and transmitted a compliant 278 admit notification.
Automated Fax Generation – for payers that required fax notifications, the system generated the appropriate documentation and transmitted it automatically via integrated fax services.
Confirmation Capture & Audit Trail – the system captured confirmation details—including fax transmittal pages—and stored them for compliance documentation and audit support.
Rules-Based Configuration
Payer-specific rules determined:
- Which notification method to use
- Required timing windows
- Required supporting documentation
This eliminated guesswork and ensured consistent adherence to payer requirements.
The Impact
