Building a Multi-EHR Quality Data Infrastructure with NCQA Data Aggregator Validation
The Challenge
A primary care association set out to consolidate quality reporting across more than 90 provider organizations operating on 17+ different EHR platforms. The initial focus was centralized HEDIS reporting, but the real challenge quickly emerged:
How do you normalize, validate, and govern data from dozens of heterogeneous clinical systems—while ensuring measure accuracy and audit defensibility?
The platform needed to ingest and reconcile:
- Daily clinical data from multiple EHR platforms
- Payer claims, care gap reports, and attribution files
- Laboratory feeds from Quest and LabCorp
- Regional Health Information Exchange (RHIE) data
As the initiative evolved, the association expanded its goals beyond reporting to include:
- Proactive care gap management
- PCP admission alerts
- Automated supplemental data submissions
- Secure payer access
- NCQA Data Aggregator Validation (DAV)
The project shifted from “building dashboards” to constructing a certifiable, scalable clinical data infrastructure capable of supporting value-based performance programs.
The Solution
We architected and deployed a cloud-based clinical data warehouse designed to ingest, normalize, validate, and operationalize data across diverse systems.
Multi-Source Data Ingestion
The platform supported a broad range of integration methods, including:
- HL7 feeds
- FHIR APIs
- Direct web services
- CCD-A extractions
- Custom payer and laboratory formats
This enabled onboarding of the most common EHR systems first, with expansion underway to smaller platforms.
Data Normalization & Validation Framework
At the core of the solution was a normalization and governance engine that:
- Reconciled inconsistent coding across EHR systems
- Validated data completeness and accuracy
- Maintained audit trails
- Supported standardized measure calculation logic
As the association pursued NCQA Data Aggregator Validation (DAV), the system was enhanced to meet stringent validation, documentation, and data governance standards required for certification.
The platform successfully supported DAV certification across participating provider organizations.
Unified Clinical Intelligence Interface
A consolidated, single-pane interface allowed users to view:
- Clinical encounters
- Claims history
- Laboratory results
- Care gap status
- RHIE queries and registry data
This eliminated EHR silos and provided a unified, actionable patient view.
Care Gap Automation & Admission Intelligence
The system extended beyond reporting into operational intelligence:
- Admission Notifications
HL7 alerts from the RHIE triggered real-time PCP notifications. - Automated Supplemental Data Submission
Monthly supplemental feeds were automatically generated and transmitted to resolve care gaps—closing thousands of gaps without manual provider intervention.
Secure Payer Access
Two major payers were granted secure, controlled access to attributed member data, eliminating repetitive provider inquiries and reducing administrative overhead.
The Impact
The platform successfully onboarded nearly 50 provider organizations across multiple EHR systems, validating the feasibility of a certified multi-source data aggregation framework.
Key outcomes included:

Following a leadership transition and a strategic shift toward formation of a Clinically Integrated Network (CIN), the broader initiative concluded. However, the project demonstrated that large-scale, vendor-neutral clinical data aggregation and validation can be executed successfully across heterogeneous environments.
