Interactive Clinical Forms

The Problem

A major health system in Canada wanted to implement interactive electronic forms as an integral part of a new Electronic Medical Record (EMR) system scheduled for deployment the following year.

Clinicians required the ability to:

  • Complete complex, specialty-specific forms digitally
  • Add notes and annotations directly onto the forms
  • Electronically sign off on each completed form
  • Automatically store the finalized document within the patient’s EMR record

However, the selected EMR vendor did not natively support this level of form interactivity or annotation functionality. To ensure the EMR could meet their clinical and operational requirements, the organization needed a prototype solution that demonstrated how these advanced workflows could be achieved — and that the EMR could be successfully extended through integration.

The Solution

Our team adapted our electronic forms automation and e-signature platform to prototype an interactive forms environment fully integrated with the target EMR.

Key Features Delivered

  1. Dynamic Form Interaction: Enabled clinicians to fill out, annotate, and sign complex forms directly within a localized interface.
  2. Bilingual Application Support: Delivered the interface and workflows in both English and French, ensuring inclusivity and compliance with Canadian language standards.
  3. EMR Integration Proof-of-Concept: Demonstrated real-time form completion and secure transmission of finalized documents into the EMR patient record. Confirmed that external intelligent form modules could complement and extend EMR capabilities without disrupting vendor workflows.

The prototype was successfully completed, giving leadership and clinicians a clear demonstration of how the new EMR could support the interactive workflows they required.

The Impact / Results

Strategic Decision-Making

  • Provided the organization with proof that the desired functionality was achievable, allowing them to confidently proceed with their EMR investment.

Clinical Engagement

  • Secured buy-in from key clinicians who were previously hesitant about the EMR’s limitations.
  • Demonstrated that custom extensions and integrations could be implemented smoothly.

Operational Readiness

  • Validated that localized, bilingual solutions could be deployed across clinical departments without introducing complexity or vendor delays.

Outcome

The health system is now moving forward with its EMR rollout in the upcoming fiscal year, equipped with the confidence that its intelligent forms and e-sign capabilities will support clinician needs and streamline documentation across the enterprise.
