Single Pane of Glass for Long-Term Care Operations
The Challenge
A long-term care facility had established strong internal best practices, but daily execution was slowed by a technology stack with limited interoperability. Staff frequently had to jump between multiple vendor applications to complete routine tasks, and there was no single source of truth that provided a complete view of a patient’s day of care.
While the internal IT team had created integration “bridges” between systems, they wanted to take the next step: deliver a true single pane of glass experience that provided a consolidated, real-time view for clinical, support, and billing teams—without replacing their existing core systems.
The Solution
We assisted with the design and implementation of a single pane of glass using a combination of technologies, centered around a real-time data hub and role-based worklists.
Real-Time Data Hub
We evaluated the best integration approach for each system and created a centralized data hub for key data elements. These elements synchronize in real time from the facility’s existing core platforms, ensuring all teams reference the same, current information.
Role-Based Daily Worklists
Using a no-code/low-code platform, we built a unified interface that presents department-specific daily worklists, allowing users to complete tasks without switching between applications.
Automated Operational Workflows
The solution standardized and automated high-impact workflows, including:
- Patient Onboarding
Onboarding packets are auto-generated and routed to onboarding staff for completion with the patient. - Order Processing
New orders are received electronically and trigger an automated workflow for review, acceptance, and prior authorization. Status and outcomes are pushed back into all core systems to maintain synchronization. - Prior Authorization Automation
Prior authorization updates are posted to multiple systems, order documentation is updated, and a provider packet is prepared and sent automatically.
Future Phase Enhancements
The architecture supports additional phases without disrupting current operations:
- Patient Transport
Automatically update transport systems with pickup time/location details for patients requiring transportation to the point of care. - Patient Tracking
Track a patient’s onsite location and make it visible within the single pane viewer to improve coordination and throughput.
With the data hub and unified viewer in place, systems remain synchronized and the interface reflects real-time updates across the patient journey.
